Our mission is to deliver exceptional holistic care throughout the journey of living with serious illness by focusing on quality of life and prioritizing the patient and family’s goals.

Palliative Care Service Overview

At Health Link Palliative Care, we offer specialized medical care wherever the patient resides (at home, in an assisted living facility, skilled nursing home, etc.) along with video visit options to ensure both the patient and family have access to the supportive services they need when they need them.
Symptom Management
One way in which we can support is by focusing on alleviating the symptoms that accompany serious illnesses. This includes managing pain, shortness of breath, nausea, fatigue, and other symptoms that affect daily life.
Planning ahead
If someone in your life has a serious illness, it can be difficult to know what the next right step is. Our palliative care team has experience and training in helping patients and families anticipate and plan for disease progression and ways to mitigate some of the frustration and undesired complications.
Emotional and Spiritual Support
Living with a serious illness can be emotionally challenging. Our team includes social workers, counselors, and spiritual care advisors who provide ongoing support to both patients and their families.
Coordination with Ongoing Treatments
Palliative care can be provided alongside curative treatments. We collaborate with your existing healthcare providers to ensure that palliative care complements and enhances your ongoing treatment plan.

Introduction to Palliative Care

Health Link of the East Bay brought my 97-year-old mother back to life following three hospitalizations. The Physical Therapist Jacob was outstanding and actually discovered the reason why my mother was collapsing when standing up! Ruta, her nurse, was dedicated and skilled in treating my mother's various difficulties with thoroughness and compassion, and my mother adored her. She even drew blood and took it to the lab.These Occupational Therapists, hygiene assistants, Physical Therapists, wound care specialists, RN's, and LVN's also brought her back to good health: Michael, Jennifer, Hannah, Joy, Jonah, Armen, Marissa, Mark and Vy. They would order medical supplies for her (which arrived the following day for our use) and make recommendations about safety devices we needed to install. They gave her individualized exercises to do between visits. They were available during emergencies, too. Mom has now returned to good health, and we are beyond grateful to Health Link!
